What is district care?

District care is care and support in your home. A district nurse or a care team helps you with what you need to continue living independently. This may be temporary - for example, after a hospital admission - or for a longer period. The district nurse works with you to see what care is needed and helps to arrange this properly, together with your network. The district team supports you in maintaining or improving your self-reliance, so that you can continue living at home as comfortably as possible.

Examples of neighbourhood care include:

• Help with washing and dressing,
• wound care or administering medication,
• advice on aids in the home,
• support for carers,
• Or help in learning ways to remain self-reliant.

The district nurse works with you to see what you can still do yourself and what help people around you can provide. Together, we make sure you get the right care.

District care in your neighbourhood

In Amstelveen, Aalsmeer and Kudelstaart, various organisations are working together to ensure the effective allocation of community care. All applications for community care in this region are processed jointly by the community care teams of:

• Amstelveen: Amstelring, Brentano and Zonnehuisgroep Amstelland
• Aalsmeer and Kudelstaart: Amstelring, ActiVite, Buurtzorg, Carpe Diem, Zorgcentra Meerlanden and Zorggroep Aelsmeer.

This collaboration will get you in touch faster with a district nurse, who will work with you to see which organisation best suits your care needs.

How to apply for district care?

1. Fill in the application form
   Go to www.wijkzorginmijnbuurt.nl/regio/amstellandmeerlanden.
2. The district nurse will contact you
   Once your application is received, it will be distributed to the district teams. A district nurse from one of the cooperating organisations will call you to make an appointment.
3. Creating a care plan together
   During the interview, you will discuss what is important to you, what you can still do yourself and what help you need. Together with you, the district nurse will make a care plan that suits your situation.
4. Launch of district care
   Once everything is fine-tuned, care starts in your home. The district nurse remains your permanent point of contact for as long as you receive district care.

Cost of district care

District care is usually reimbursed through your basic insurance or via the Long-term care act (Wlz), depending on your situation and care needs. Sometimes a personal contribution, e.g. for personal care or aids. This differs per situation and organisation. The district nurse or the district team can explain which costs apply to you and whether you are entitled to reimbursements or subsidies.
