Registering with S. Derks General Practice

New patients are welcome at S. Derks General Practice. You can easily register via MyPractice.co.uk. You can register with us if you live in Amstelveen (postal code area 1181 and 1182).

Practical information about your registration

• When you visit us for the first time, we kindly request that you bring valid identification (passport or ID card) and your insurance details.
• By registering, you give us permission to deregister you from your previous GP. We also request that you inform your current GP that you are switching.
• You are, of course, welcome to visit us first for an introductory meeting. To do so, please contact our assistant by telephone to make an appointment.

Please note: completing and submitting the registration form does not automatically mean that you are immediately registered. After we have received your form, we will contact you as soon as possible to confirm whether registration is possible.
